Collected Personal Data
Personal data is collected for insurance purposes and for health and safety. Contact details for yourself including name, email and telephone numbers are taken to make appointments or to contact you to change your appointment. Your next of kin details are stored in case of emergency. Dates of birth are taken to make sure I have the correct patient and in case of the need to contact your GP. Your GP information is also gathered as this is a mandatory requirement in the British Acupuncture Code of Professional Conduct.
Consent on the pre first appointment form /initial consultation form gives your permission for your data to be held on paper or electronically. Patient consent records are kept as evidence that a patient has consented to treatment.
This privacy notice applies to information I collect from potential patient enquiries via the website form, current and former patients.
Please note that I do not use encrypted emails so any emails I send or receive may not be protected in transit. This includes the pre first appointment form which may be emailed by the patient.
Appointment history is kept electronically for tax purposes, law, insurance purposes or complaint to professional body.
In order to formulate a treatment plan and make a traditional Chinese medicine diagnosis, your medical history, symptoms and medical complaints are recorded.
I record your treatment given and any advice given for law and insurance purposes and for referral to other practitioner purposes.
Any adverse incident will be reported to the Association of Acupuncture Clinicians and insurer to help with potential claims and research data/safety guidelines.
